DESCRIPTION OF SUPPORTED LIVING SERVICES

Providing individually tailored services and supports that enable a person to live in his or her own home. The type and intensity of support may vary widely depending upon the person's unique needs and abilities. The home is under the control of the individual(s) and/or his or her family, conservator, or other representative rather than the provider of services and supports. The selection and control of the service is provided by the persons receiving services with the assistance of the support coordinator and/or family when necessary.
